New Client Registration Process:

  1. Please review the Credentialing Agreement
  2. Complete the Registration form (below)
  3. Make a Payment when registering as an Individual, or you will receive an Invoice when registering as a Group
  4. We contact you by automatic email to obtain your credentials information
  5. You send us your credentials information (license details, memberships, continuing education history, etc)
  6. We perform initial data recording on your account at this site
  7. You receive monthly reports and login credentials to manage your data through this site

Make a Payment (Individual)

Individuals make a secure card payment of:

  • First year service fee: $360 (recurring annually)
  • Initial setup services: $100 (one-time)
  • Total charge today: $460

Note: When registering as a group of 2 or more licensed professionals, you will receive an itemized invoice.
